Funeral Homes that order certificates from more than one Vital Records Agency are required to select an Agency before creating an order. |

Agency selection drop down is displayed upon login

Select Vital Records agency from which to order the certificates from the available drop down list

| Step 4 |
Click Next button to confirm
|
| Step 5 |
Decedent data screen displays. Orders will be sent to selected Vital Records Agency (displayed in Order Status line)
